Key Responsibilities:
- Assess new referrals to determine the support needed, helping customers maintain independence with respect to their dignity and confidentiality.
- Guide customers in managing their homes effectively.
- Assist with maximizing benefits, budgeting, managing debts, and handling forms, while connecting them to other agencies as needed.
- Help set up tenancies and source essential household items.
- Facilitate access to services from external agencies tailored to individual needs.
- Develop and regularly review personalized support plans with customers.
- Maintain weekly contact through visits, calls, emails, and texts.
- Keep accurate and up-to-date support files and write detailed support plans.
Housing Management:
- Ensure compliance with relevant legislation and internal health and safety procedures.
- Proactively safeguard customers by identifying and supporting those at risk, following strict safeguarding protocols.
- Work collaboratively with other teams to ensure consistent support across the organization.
